Hyogo Medical University 2024 Body Donation Registration
Our university accepts body donation registrations over the phone.
If you would like to register, please read the following points and contact the university.
Please note that documents will not be distributed at the counter.
1. Conditions for body donation registration
Area: In principle, residents of Hyogo Prefecture
Age: 20 years old or older
Consenters: Two or more relatives (acquaintances and friends are not accepted, and relatives must be related by blood to the sixth degree or by marriage to the third degree)
2. Steps to complete registration
1. Call the Nishinomiya Educational Affairs Division, Body Donation Section. (Please call in person.)
2. A booklet about body donation and a body donation registration application form will be delivered to your home.
3. Read the booklet carefully and discuss it with your relatives.
4. Fill out and sign the body donation registration application form and return it.
5. You will receive a donation registration certificate at a later date (registration complete).
